To enroll at Enterprise Junior High School please provide
- Proof of identification of parent/legal guardian
- Proof of custody (signed by a judge) if student lives with someone
other than both natural parents (example - Divorce papers stipulating who
has physical custody of minor children)
- Student's birth certificate
- Student's social security card/ military ID
- State of Alabama Immunization Form (Must be presented at time of
registration)
- Withdrawal form (with grades) from previous school or last report card
if beginning of academic year
- Two proofs of evidence of residence (utility bills, drivers license,
lease or rental agreement, etc. . .)
- Discipline/attendance records from previous school, if applicable
